What is Sub Mental LipoSelection?

Sub Mental (under the chin) LipoSelection is a surgical technique to improve the neck line by removing fatty deposits and is often performed in conjunction with neck, face lift and chin implant surgery. Fat in the sub mental region often can be related to being overweight. However, even in individuals of normal weight, fat may be present. As aging progresses, many people develop sub mental fat deposits independent of their weight. This may be more apparent as the skin loses elasticity and begins to hang.

Sub Mental LipoSelection is not an alternative to weight loss but can remove stubborn fat deposits which do not respond to diet and exercise.

Sub Mental LipoSelection can be performed under “twilight anaesthesia” (which is local anaesthetic with heavy sedation) or under oral sedation.


What will Sub Mental LipoSelection Achieve?

The aim of the Sub Mental LipoSelection is to remove excess fat deposits from under the chin. A well planned contouring procedure will leave you with a more defined side profile and looking fresher from a front viewpoint.


Who should have Sub Mental LipoSelection?

As with all elective surgery, good health and a realistic expectation are prerequisites, but if you want to remove excess fat from under your chin sub mental liposuction may be appropriate especially if:

  • Your surgeon is aware of any medical conditions, allergies you may have and whether you are a smoker. Also all medications including supplements you are taking.
  • You are not prone to scarring problems such as Keloid.
  • You have specific fat pockets that are resistant to diet and exercise and desire re shaping, not weight loss
  • You do not have frequent fluctuations in weight
  • You have had no previous surgery to the area where liposuction is desired.
  • You have good facial elasticity.
  • You have a good understanding of the limitations of the procedure.

It works as an excellent stand alone procedure or in conjunction with a lower face lift or Contour Threads.


How is the procedure performed?

There are variations to the procedure, but the typical will be described.

The procedure is a day stay and performed under intravenous sedation by our resident anaesthetist. Once anaesthetised, the procedure will take approximately 60 minutes.

A Tumescent solution containing normal saline mixed with local anaesthetic and adrenaline is injected with minimal discomfort directly into areas of excessive fatty deposits. The Tumescent technique for liposuction provides local anesthesia to the area to be treated constricts capillaries to prevents surgical blood loss and makes the fat deposits easier to break down and be extracted. This extra fluid also minimizes trauma to the surrounding tissue, reducing swelling and post-operative pain.

Once the liquid is injected, a small incision will be made in the skin, usually placed in a natural skin crease under the chin or behind the ear lobes. A thin tube (called a cannula) connected to a vacuum is inserted into the fatty layer. Using to and from movements, the fat is drawn through the cannula into a collection system. This action breaks up the fat deposits and sculpts the area to the desired proportions. The unwanted fat is removed with a high pressure vacuum, leaving the skin, muscles, nerves and blood vessels intact. An elastic compression garment is worn for 24 hours during the first week and 12 hours in the second and third week. The longer you wear your garment the better compression and contouring you will achieve.

Most patients are seen the next day, and the incision lines redressed. 24 hours after the procedure, patients can remove their garment to wash their hair and apply the prescribed antibiotic ointment. Sutures are removed after day 7.

The application of topical Arnica cream can aid the healing process, along with following your post-operative instructions carefully.


How will I be cared for on the day of my surgery?

You will be admitted to the Skin Institute approximately 1 hour prior to surgery. Although extensive pre-operative checks will have already been done, this is time for last minute questions with your surgeon and the anaesthetist. You are changed into our gowns and then placed in bed with a pre-med. After the procedure there is a recovery time of 1 hour when you will be given something to eat, dressings will be applied and instructions given ready to go home. It is not typical to need hospital admission but this can be arranged at the patient’s request.

 

What do I do once I'm home?

You will be permitted home within approximately 90minutes following your procedure with a person of your choice that must stay with you for a minimum of 24 hours. There will be swelling and sometimes bruising. It is strongly recommended you sleep propped up for 48 hours to aid in the reduction of swelling.

A soft diet is recommended as the lower jaw and neck may be tender for a few days.

It is important to avoid any over activity, bending over, alcohol consumption and vigorous activity for a week. It is important to take your medications as prescribed. Most patients are able to return to work after two to three days.


How will I look and feel after the surgery?

By the end of the first week your face may still be swollen. Dressings will be removed so you can shower and sutures will be taken out. You can wear make up to conceal any discolouration.

By the end of the second week most of the bruising will have disappeared, sutures will have dissolved and you can start to think about resuming normal activities including non strenuous lower body work outs.

By one month you may resume exercise; your new chin or neck will start to settle. The healing process will be well under way helping with the contraction of the tissues. You may notice tingling as numbness around the chin area subsides, although numbness may persist for several months.


Where do I go from here?

Before your surgery you will need a consultation with your surgeon to assess your suitability for the procedure and to discuss possible undesired effects and contraindications, including bruising, numbness, lumps and swelling. You will also have a consultation with a cosmetic nurse to prepare you for the surgery and discuss any concerns.
